Recommendations:
1. Do not administer or have administered the Leptospirosis vaccine to Dachshunds. They are susceptible to severe allergic reactions to this vaccine.
2. Dachshunds are prone to skin issues. Please use a high quality dog food free from corn, wheat or soy. Grains such as brown rice, oats and barley are acceptable.
3. Coconut oil is a natural anti-fungal and anti-bacterial. It can be used topically for wounds, rashes or lesions. It can also be used on the dog’s food. Dogs typically enjoy the taste of coconut oil. If your dog is a finicky eater, it will encourage them to eat.

This puppy was seen at WILLAMETTE VALLEY ANIMAL HOSPITAL GLADSTONE and no health concerns were present. A copy of those findings are included.
This puppy is registered with and transferred with:

The breeder cannot guarantee size, show quality or breeding/training ability of this puppy. This puppy is being sold as pet quality only. The breeder makes nor implies any warranties or guarantees, express or implied of the suitability of the puppy to the adopter and/or his/her family other than what is in this agreement. I hereby understand and agree the breeder makes no representations or warranties, expressed or implied, about the above mentioned animal's temperament and is hereby absolved from any liability for future damages or injuries caused by said animal.
I, ___________________________________________, hereafter referred to as the adopter, hereby agree that the above described animal is being adopted by me solely for myself and/or my immediate family. I agree that will not sell, give away or otherwise dispose of said animal to any person(s), dealer, retailer, auction, institute, pet leasing company (in particular the Hannah Pet Society) or any other entity for any reason without breeder’s consent. If at a later date I am unable or unwilling to keep this pet, I agree to first contact the above named breeder and give them the option to reclaim said pet at no charge to breeder.
I hereby agree to care for the above described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water and veterinary care. I further agree that said pet shall reside inside my home and shall not be allowed to roam freely.
I hereby agree I will have 3 business days to have the puppy seen by a veterinarian. If I choose not to have the puppy examined by a licensed veterinarian I agree that I accept the puppy “as is” at the time of sale.
I hereby understand and agree that breeder will issue a 2 year health guarantee against life threatening congenital disorders. To keep this warranty valid, the dog must be examined by a licensed vet within 72 hours (excluding Saturday and Sunday) of receipt of the puppy. If the vet finds dog to have a congenital life threatening disorder, this dog must be returned immediately to the breeder, (within 48 hours) accompanied by a certificate statement from the vet for replacement. If a replacement puppy is not available at the time of return, one will be made available as soon as reasonable. There are absolutely NO REFUNDS! All veterinary expenses are the responsibility of the BUYER. Said Puppy is guaranteed against life threatening congenital disorders for up to (2) two years and will be replaced with puppy of equal value when available.
I further understand and agree, viral illnesses, common parasites, infections, improper bites, fleas, ticks, worms, mites, COCCIDIA, GIARDIA or Kennel cough etc. are not cause for replacement as they are not life threatening congenital disorders. The health guarantee does not cover illness due to ingestion of foreign objects, food, chemicals, stress related issues, behaviors issues, physical injuries and/or illnesses which may have occurred and/or contracted during transport or illnesses contracted from contagions. The breeder will NOT pay associated vet bills that may occur from any of these things regardless if puppy had any of these things at the time of purchase and possession. We makes every effort to make sure that your puppy is healthy and free of parasites. Many of these things are a result of stress with going to a new home and do occur when puppy has otherwise been healthy at breeders. Most all puppies have worms and you should not be surprised if your puppy does have them. Although we worm on a regular schedule every two weeks as the vet recommends, we can't guarantee that your puppy will be free of all worms when you get it.
I certify that all statements made by me on this adoption/sale agreement are true and correct. I agree the breeder has the right to confiscate the above described animal in the event that any statements made by me are found to be false.
From time to time the breeder is asked for a list of references. Do you consent to allow breeder to share your email address only? _____ Yes _____ No.
